Your new role
You will provide a high quality and responsive accommodation based support service in order to assist service users to develop the personal and social skills that will enable them to maintain their accommodation, improve their health and wellbeing, improve their employability and financial inclusion and decrease any negative impact of their behaviour within/on their locality.

The main responsibilities for the role will be:

* To carry out detailed needs assessments of service users during coming into the service
* To work with service users to develop an outcome focused support plan to enable them to access and maintain appropriate accommodation
* To carry out detailed assessments of the needs of service users during their stay at the project
* To work with service users to ensure a quick throughput of service users and smooth transitions between relevant services
* To facilitate mediation sessions with service users and their families/carers with the aim of them returning to live at home and preventing them becoming entrenched in supported housing services
* To support service users to identify and manage any risks identified as part of the application and support process
* To enable service users to understand the terms and conditions of their Licence Agreement support service users in improving their financial wellbeing, by advising them of the availability of benefits to which they may be entitled, progressing their claims and supporting them to claim and maintain benefits where appropriate.
* To maintain accurate detailed records of the support programme and interventions provided and to produce reports as required on the progress and outcomes achieved by service users.
