We are working with a charitable organisation to source a temporary to permanent Mental Health Support Co-ordinator to provide a high-level service to those needing the support within the community. This role will be working from home, with occasional requirement to attend the office in East Kent.
This post is vital to the delivery of the service contracts and makes an important contribution to the range of services provided across East Kent and will be responsible for providing appropriate up to date information, advice, guidance, support and assessment, specifically but not exclusively, supporting someone with a mental health illness.
The role
* Provide emotional support (not counselling) and information and advice to support someone with a mental health illness
* Identify needs, and support options to meet those needs
* Build a rapport and maintain regular contact
* Liaise with other organisations and agencies
* Process and make referrals to and from other agencies (both statutory and non-statutory)
* Complete statutory Carers Assessments, review assessments and support plans in line with Policy, the Care Act and organisational procedures
* Assess eligibility to access funded support
* Provide advice and guidance in relation to benefits and link to practical assistance to complete associated forms
* Plan, organise and facilitate support groups, activities and events
* Carry out risk assessments as required
